Sounds like corrupted game date. Try verifying the integrity of the game cache files. verifying cache did nothing for me. Are you using a sorting mod? Fallout 4. So I installed Def_UI and now I get the "Lookup Failed". I tried uninstalling it but that didn't fix anything. I also tried to get steam to restore my files, but that did nothing.
